License renewal to operate a facility serving people with developmental disabilities, also know as an intermediate care facility for individuals with intellectual disabilities (ICF/IID).




License required to operate a residential facility for four or more unrelated persons with developmental disabilities. Treatment is provided to help these people maintain and improve their current skills and to develop new skills (e.g., work, social, activities of daily living, education, etc.).


Does not expire.


$6 per bed fee annually and for change of ownership.

An annual form will be sent by the licensing agency. If you require a duplicate, contact the Division of Quality Assurance at 608-266-6646 or via email.
