Statutorily Required Reports

The Wisconsin Department of Health Services (DHS) is statutorily required to create one-time, quarterly, annual, and biennial reports on behalf of the agency. DHS is required to provide these reports and information to the legislature and governor. DHS is also required by statute to post reports and information created by partners to its website.

Wisconsin Stat. § 51.06(8)(b)

(a) In this subsection: 1. “Intermediate care facility for individuals with intellectual disabilities” has the meaning given in 42 USC 1396d (d). 2. “Medical assistance programs” has the meaning given in Wis. Stat. § 49.43 (8). 3. “Nursing home” has the meaning given in Wis. Stat. § 50.01(3). (b) Annually by October 1, the department shall submit to the joint committee on finance and to the appropriate standing committees of the legislature under Wis. Stat. § 13.172(3) a report that includes information collected from the previous fiscal year on the relocation or diversion of individuals who are Medical Assistance eligible or recipients from nursing homes, intermediate care facilities for individuals with intellectual disabilities, and state centers for the developmentally disabled.

By March 1 of each year, the department shall submit a report to the joint committee on finance and to the chief clerk of each house of the legislature, for distribution to the appropriate standing committees under Wis. Stat. § 13.172(3), describing the program's impact during the preceding calendar year on state employees, including the department's efforts to redeploy employees into vacant positions and the number of employees laid off

Section 1. 51.42 (7) (d) of the statutes is created to read: 51.42 (7) (d) By January 1, 2015, and by January 1 of each odd-numbered year thereafter, the department shall submit to the legislature under s. 13.172 (2) a report that describes mental health services and programs provided by counties and regions comprised of multiple counties.

Healthy Food Incentive Program

Wisconsin Stat. § 49.79(7w)(d)

Beginning no later than March 24, 2025, and annually thereafter, the department shall submit to the chief clerk of each house of the legislature, for distribution to the appropriate standing committees of the legislature under s. 13.172 (3), a report regarding implementation of the program under this subsection, including data collected relating to funding and participation and any other information necessary to evaluate the program.

Wisconsin Stat. § 46.03(26)

The department shall submit a report each December 31 to the Joint Committee on Finance and to the Chief Clerk of each house of the legislature, for distribution to the appropriate standing committees under Wis. Stat.§ 13.172(3), regarding the data processing projects under development.

Newborn Hearing, 2022

Wisconsin Stat. § 253.115

"Beginning July 1, 2002, the department shall annually collect information from hospitals for the previous calendar year concerning the numbers of deliveries in each hospital and the availability in each hospital of a newborn hearing screening program. From this information, by July 31, 2003, and annually thereafter, the department shall determine the percentage of deliveries in this state that are performed in hospitals that have newborn hearing screening programs...."

Gifts, Grants, Bequests, or Devises 2024 Report

Wisconsin Stat.§ 20.907(1m)

Reporting. State agencies shall, by December 1 annually, submit a report to the joint committee on finance and the department of administration on expenditures made by the agency during the preceding fiscal year from nonfederal funds received as gifts, grants, bequests or devises.

Wisconsin Stat. § 153.05(2s)

(2s) Annually, the department of health services and the department of employee trust funds shall jointly prepare and submit under s. 13.172 (3) to standing committees of the legislature with jurisdiction over health issues a report on the activities of the data organization under this chapter.

2024 Medicaid Dental Pilot Program Evaluation

Wisconsin Stat. § 2.49.45(24k)(c)

No later than January 1, 2020, and biennially thereafter, that the Department submit to JFC, the chief clerks’ offices and to the health or public benefits committees a report that identifies the number of MA members who received services under the pilot program, an estimate of the potential reduction in health care costs and ER visits due to the pilot program, the feasibility of continuing the pilot project and expanding in specific areas of the state or statewide, the amount of money distributed under the project and an explanation if not all funds were distributed, and an analysis of Medicaid member populations who received services under the pilot program and populations who were not enrolled but who may benefit from participation.

Statewide Immunization Program 2023 Report

Wisconsin Stat. § 252.04(11)

Annually, by July 1, the department shall submit a report to the legislature under § 13.172(3) on the success of the statewide immunization program under this section.

Hunger Report, 2022

Wisconsin Stat. §§ 49.76(4) and (5)

(4) Develop an annual plan that documents areas of hunger and populations experiencing hunger within this state and that recommends strategies and state and federal policy changes to address hunger in these areas and populations. (5) Submit, by December 31 annually, the plan developed under sub. (4) to the governor, superintendent of public instruction and the appropriate standing committees under s. 13.172 (3).​

Tobacco Use Cessation: 2023 Annual Report to the Governor and the Legislature

Wisconsin Stat. § 255.15(4)

(4) Reports. Not later than April 15, 2002, and annually thereafter, the department shall submit to the governor and to the chief clerk of each house of the legislature for distribution under § 13.172 (2) a report that evaluates the success of the grant program under sub. (3). The report shall specify the number of grants awarded during the immediately preceding fiscal year and the purpose for which each grant was made. The report shall also specify donations and grants accepted by the department under sub. (5).

Wisconsin Diabetes Action Plan Report to the State Legislature, 2024

Wisconsin Stat. § 255.085

(1)  The department shall, in consultation with the department of employee trust funds, develop and implement a plan to reduce the incidence of diabetes in Wisconsin, improve diabetes care, and control complications associated with diabetes. The department may also consult with the department of public instruction and the department of corrections in developing a plan under this section.

(2) By January 1, 2021, and biennially thereafter, the department shall submit a report to the chief clerk of each house of the legislature for distribution to the legislature under §13.172(2)

Wisconsin Stat. § 253.12(4)(d)

Opioid and Methamphetamine Treatment Programs Progress Report to Legislature: 2024 Report

2013 Wisconsin Act 195

By the first day of the 24th month beginning after the effective date of this subsection, April 8, 2014, and annually thereafter, the department shall submit to the joint committee on finance and to the appropriate standing committees under Wis. Stat.§ 13.172(3) a progress report on the outcomes of the program under this section.

Section 1. 20.435(5)(bc) of the statutes is amended to read: Grants for community programs. The amounts in the schedule for grants for...opioid treatment programs under Wis. Stat. § 51.422.

Intensive Care Coordination Program

Wisconsin Stat. § 49.45(26g)(4(h)

Requires the Department to submit to JFC a report to JFC summarizing information reported by participating hospitals and health care systems, including costs savings due to avoiding emergency department visits. This report must be submitted no later than 24 months after the date on which the first hospital or health care system begins enrolling individuals into the intensive care coordination program

Report on Rehabilitation Requests, 2023

Wisconsin Stat. § 50.065(5g)

(5g) Beginning on January 1, 1999, and annually thereafter, the department shall submit a report to the legislature under Wis. Stat. § 13.172 (2) that specifies the number of persons in the previous year who have requested to demonstrate to the department that they have been rehabilitated under sub. (5), the number of persons who successfully demonstrated that they have been rehabilitated under sub. (5) and the reasons for the success or failure of a person who has attempted to demonstrate that he or she has been rehabilitated.
