License continuation to operate a hospice.




A licensed agency that provides care to terminally ill persons who have a life expectancy of 6-12 months. This care can be provided in the person's home or in another setting (e.g., nursing home, community based residential facility, etc.)


Non-expiring. However, agencies are required to pay the annual fee and submit an annual report on a schedule determined by the Division of Quality Assurance.


Annual fee - amount equal to 0.15% of the net annual income. The annual fee is no less than $200 and no more than $1,000.


Already licensed as a hospice.
