Description

A hospice license is for an agency that cares for people who have a terminal illness. Those who get hospice are often expected to live six to 12 months. Hospice care can happen at home or in another setting, such as a nursing home.

Fees

$300 one-time application fee for a provisional license. For permanent hospice licenses, yearly fees are 0.15% of the agency's net annual income. They range from $200–$1,000.
