Scam targeting U.S. Bank customers
DHS is aware of an organized impersonation scam that targets U.S. Bank customers. Impersonators are contacting customers by phone, posing as representatives from the U.S. Bank Fraud Department. They may claim issues with recently initiated wire transfers. The DHS DQA Provider Portal uses U.S. Bank for the payment of fees. Please be aware of this impersonation scam if you are paying fees through the DHS DQA Provider Portal.

Description
A community support program for chronically mentally ill persons means a coordinated care and treatment program, which provides a range of treatment, rehabilitation and support services through an identified treatment program and staff to ensure ongoing therapeutic involvement, individualized treatment, rehabilitation and support services in the community for person with chronic mental illness.
Duration
Up to two years with an annual fee.

Fees are paid annually.

Instructions
The Division of Quality Assurance (DQA) sends desk or site renewal information at least 60 days prior to expiration. These materials are to be returned within 30 days of expiration. If you require duplicate information, contact the DQA Behavioral Health Certification Section.
The following applications are required for an community support program for chronically mentally ill persons program recertification:
- Community Support Program (CSP) for Persons with Chronic Mental Illness Initial Certification Application - DHS 63, F-00546
- Behavioral Health Services Recertification Application - DHS 94 Patients Rights and Resolution of Patient Grievances DHS 92 Confidentiality of Treatment Records, F-00276 (completed once per certificate annually)
