Scam targeting U.S. Bank customers

DHS is aware of an organized impersonation scam that targets U.S. Bank customers. Impersonators are contacting customers by phone, posing as representatives from the U.S. Bank Fraud Department. They may claim issues with recently initiated wire transfers. The DHS DQA Provider Portal uses U.S. Bank for the payment of fees. Please be aware of this impersonation scam if you are paying fees through the DHS DQA Provider Portal.

Description

A hospice license is for an agency that cares for people who have a terminal illness. Those who get hospice are often expected to live six to 12 months. Hospice care can happen at home or in another setting, such as a nursing home.

Fees

$300 one-time application fee for a provisional license. For permanent hospice licenses, yearly fees are 0.15% of the agency's net annual income. They range from $200 to $1,000.
