Scam targeting U.S. Bank customers

DHS is aware of an organized impersonation scam that targets U.S. Bank customers. Impersonators are contacting customers by phone, posing as representatives from the U.S. Bank Fraud Department. They may claim issues with recently initiated wire transfers. The DHS DQA Provider Portal uses U.S. Bank for the payment of fees. Please be aware of this impersonation scam if you are paying fees through the DHS DQA Provider Portal.

The Wisconsin Department of Health (DHS) Division of Quality Assurance (DQA) launched the DHS DQA Provider Portal, an online system for licensure/certification, background checks, plan reviews, and other provider applications for certain users. This webpage has answers to frequently asked questions about the DHS DQA Provider Portal.

How do I create an account for the DHS DQA Provider Portal?

To create an account, choose the "Sign Up" option and follow the steps with the account creation process. Please watch the video “Account login creation” for more information.

How do I use my existing account to login to the system?

If you are logging in to the DHS DQA Provider Portal for the first time, you will need to set up an account. If you are a returning user, you can use the login credentials created when you set up your account. Please refer to the “Account login creation” video for more information.

How do I create a new authorized representative?

To establish a new authorized representative, you can either search for an individual in the system using the individual search command or create a new individual record in the system by using the new individual command on the manage authorized representatives page within the provider dashboard. Refer to the “Creating an authorized representative” and “How to Edit an Authorized Representative” videos for more information.

Will I be able to submit required licensing documents in paper form?

The DHS DQA Provider Portal requires that all documents be submitted electronically. You must upload your licensing documents to the DHS DQA Provider Portal. Please refer to the “How to Submit Licensing Requirements as Documents” video for more information.

What web browsers are compatible with the DHS DQA Provider Portal?

Firefox, Edge, and Chrome are supported by the DHS DQA Provider Portal.

What happens if there are lags?

If you encounter issues with pages or images failing to load, or if there is a delay in loading times, try clearing your browser's cache and cookies.

What file sizes and formats are acceptable for uploads?

The DHS DQA Provider Portal allows document uploads with a maximum file size limit of 100 MB.

Supported file formats include pdf, doc, odt, xls, xlxs, mp3, m4a, ogg, wav, mp4, m4v, mov, wmv, avi, mpg, ogv, 3gp, 3g2, jpg, jpeg, png, and gif.
