IRIS Consultant Agencies

What is an IRIS consultant agency (ICA)?

If you participate in IRIS (Include, Respect, I Self-Direct), you will work with an IRIS consultant agency. Your aging and disability resource center (ADRC) will offer you options, and then you can choose which agency you want to work with. The ICA helps IRIS participants manage their own care. They also handle the day-to-day operations of the IRIS program.

When you enroll in IRIS, you will choose which ICA you want to work with. The agency will:

  • Give you a welcome phone call.
  • Help you choose (or assign you) an IRIS consultant. If you ever want to change your IRIS consultant, you can contact your ICA to request it.
  • Schedule your first face-to-face meeting with your consultant.

What is an IRIS consultant?

An IRIS consultant is a person from the ICA who knows about services in your area. They will help you:

  • Understand IRIS requirements.
  • Learn the rules you must follow.
  • Complete paperwork.
  • Answer your questions.

You and your consultant will work as a team to discover your goals and long-term support needs. The consultant will also help you create your plan of care. Your plan must be approved before you can begin receiving services or supports. You will meet with your IRIS consultant regularly to make sure you are getting the help you need.
