Ambulatory Detoxification Service is certified by the Department of Health Services under Wisconsin Administrative Code Chapter DHS 75.08.

Description
An ambulatory detoxification service is a medically managed or monitored structured detoxification service on an outpatient basis, delivered by a physician or other service personnel acting under the supervision of a physician. Annual renewal of all services and programs are required for certification. If you chose to end this service please notify us in writing to the address above with the specific end date.

Up to 2 years with an annual fee.

Prerequisites
Already certified as an ambulatory detoxification service.
Instructions
The Division of Quality Assurance sends Desk or Site renewal information at least 60 days prior to expiration. These materials are to be returned within 30 days of expiration. Annual renewal of all services and programs are required for certification. If you chose to end this service, please notify the Behavioral Health Certification in writing.

*These should be completed once per certificate annually.
