ADRC Professionals: Family Care at the Non-Nursing Home Level of Care

Your ADRC (aging and disability resource center) or Tribal ADRS (aging and disability resource specialist) will let you know if you are eligible for services at the non-nursing home level of care. They use a functional screen to determine your level of care based on your needs. At the non-nursing home level of care, you have the option to enroll in a reduced-benefit Family Care program.

With Family Care, a care team will help you find and schedule services. These services are cost-effective alternatives to services you normally pay for with your Medicaid ForwardHealth Card. Family Care services at the non-nursing home level of care are more limited than services at the nursing home level of care.

Family Care services

You may receive these Family Care services at the non-nursing home level of care:

• Alcohol and other drug abuse day treatment services (in all settings).
• Coordination of care, including access to a social worker, and registered nurse.
• Home health care.
• Medical supplies.
• Mental health day treatment services (in all settings).
• Mental health services (does not apply if you need a medical doctor or are admitted to the hospital).
• Skilled nursing.
• Occupational therapy, physical therapy, and speech and language pathology services (does not apply if you’re admitted to the hospital).
• Personal care.
• Medical equipment, except hearing aids, and prosthetics.
• Medical transportation, except with an ambulance, or common carrier.
