ADRC: Help Making Long-Term Care Decisions for Older People and Adults with Disabilities

Aging and disability resource centers (ADRC) can help people decide whether to move from home. This service, called pre-admission consultation (PAC), helps older people and adults with disabilities—and the loved ones who help them—understand their long-term care options.

That may include:

• Moving into a nursing home or an assisted living facility.
• Staying at home with in-home services put in place.

By talking to an ADRC specialist, you can learn:

• Available options.
• If you qualify for government funding.
• The costs of your options.

For more information on PAC, or to find your local ADRC, find your county or Tribe's PAC brochure on the table below. The brochure is available in English, Spanish and Hmong. If you need PAC in a language other than Spanish, English, or Hmong, contact the ADRC or Tribal aging and disability resource specialist in your service area to request language translation or interpreter services.

Long-term care facilities are required to give a copy of this brochure to prospective residents.
