  Basic dental: Two visits a year with exams, cleanings, x-rays and fluoride treatments for no cost

  Extra dental: $1,000 towards non-routine diagnostic, and restorative services, endodontics, periodontics, and extractions.

  Vision:

  • One eye exam per year for no cost
  • Up to $500 for contact lenses or glasses glasses a year

  Hearing:

  • One routine hearing exam per year
  • $2,000 towards two covered hearing aids every three years

Wellness programs: Credits towards fitness services

Other:

  • $55 a month towards covered over-the-counter items
  • 28 meals following a hospital discharge
  • Transportation: 40 one-way trips to approved locations, up to 60 miles
  • Podiatry: Unlimited routine foot care visits for 20% coinsurance
