IRIS Participant Toolkit for Adult Long-Term Care Provider Enrollment
All IRIS vendors have to enroll with Wisconsin Medicaid and get a Medicaid provider ID to keep delivering your services in 2026. Before, IRIS vendors worked with fiscal employer agents to enroll with Medicaid. Now, they have to go to the ForwardHealth Portal and self-enroll as a Medicaid provider there. They must enroll before January 1, 2026, to avoid disruptions in the new year.
We need your help. 2026 is closer than it seems, and time is running out for vendors to enroll. IRIS participants should tell their vendors to enroll right away and share resources with them.
Please use the following resources to talk with vendors and give them materials to help them enroll.

- Individuals who are not participant-hired workers
Please note: participant-hired workers do not have to enroll with Wisconsin Medicaid. The requirement does not apply to them so they will keep working as usual.
