IRIS Participant Toolkit for Adult Long-Term Care Provider Enrollment
Your providers are the people, organizations, and companies that do things you need help with, also called services, through the IRIS (Include, Respect, I Self-Direct) program. Your providers are paid by Wisconsin Medicaid for IRIS services. Wisconsin Medicaid has changed how your providers sign up to be able to provide services to you:
- Participant-hired workers (PHWs) do not need to sign up. They will keep working as usual. PHWs are workers you sign or approve timesheets for.
- All of your other providers must start signing up to be a Medicaid provider using the ForwardHealth Portal by December 31, 2025.
If your providers don’t sign up now, they won’t get paid to deliver your IRIS services starting April 1, 2026. Ask your providers if they are planning to sign up to provide Medicaid services. If they don’t, that means you’ll have to find new providers before April.
We need your help! You should:
- See what types of providers you have. Check your IRIS Individual Services and Supports Plan (ISSP). PHWs do not need to do this.
- Tell your providers they need to start signing up by December 31, 2025.
- Share resources with your providers. Find them in this toolkit.
- Have a plan before April 2026 if any of your providers won’t sign up. If you have already spoken with your provider and they said they do not plan to sign up, ask your IRIS consultant for help finding a new provider.
