Wisconsin State PATH Provider Manual
This manual is designed to help agencies understand the nuances and complexities of the Projects for Assistance in Transition from Homelessness (PATH) program. It offers tips on actualizing, implementing, and translating PATH program services to urban and rural environments.
The PATH program is the only federal initiative designed for practicing outreach and engagement with people who are experiencing homelessness or at imminent risk of homelessness and who have a serious mental illness or serious mental illness and substance use disorder, which is often referred to as co-occurring disorders. Individuals who are eligible for the PATH program are among the most vulnerable in their communities, and it takes a special skill set to build trust and foster the kind of relationships that can ultimately lead people from homelessness to sustainable lives in the community.
The purpose of this manual is to promote consistency with legislative requirements and among providers implementing the PATH program across Wisconsin. It includes practical explanations, insights on how the information can be used, and client success stories.
