Eligibility Management: Forest County
At the Wisconsin Department of Health Services (DHS), we are dedicated to serving people across the state. We help those who are eligible for benefits through our state and federal programs. These programs include Medicaid, BadgerCare Plus, FoodShare, and others.
We partner with county and Tribal offices across the state to help you find out if you’re eligible for our programs. This includes helping you:
- Apply, renew, or make changes to your benefits.
- Interview for FoodShare benefits.
- Get answers to your questions.
There is a new phone number for Forest County members to call for help with state benefits
Starting January 1, 2026, you’ll have a new phone number to call for help with your Wisconsin state benefits: 833-319-3091. The new Milwaukee Enrollment Services (MilES) Forest County Unit will provide services for people who live in Forest County. The unit supports programs like Medicaid, BadgerCare Plus, FoodShare, and the Wisconsin Shares child care program.
Note: The Forest County Potawatomi Community and the Sokaogon Chippewa Community offices in Crandon will continue to provide services for Tribal members. Their locations and phone numbers remain the same.
