American Rescue Plan Act: Medicaid HCBS Rate Reform Initiative

Wisconsinites with disabilities and who are older rely on HCBS to meet their daily needs. HCBS are a cost-effective alternative to higher-cost institutional services, such as nursing home care or hospital services. Demand for HCBS is growing, and the state needs higher HCBS rates to recruit and retain direct care workers.

The Wisconsin Department of Health Services (DHS) has used ARPA funding to roll out a 5% rate increase and create a new HCBS minimum fee schedule.

New HCBS minimum fee schedule

We launched a new HCBS minimum fee schedule (opens external link) on October 1, 2024, which defines the lowest rates managed care organizations (MCOs) can pay providers of certain adult long-term care services in Family Care, Family Care Partnership, and the Program of All-Inclusive Care for the Elderly (PACE). This applies to:

  • Supportive home care services
    • Agency
    • Member self-directed
  • Residential facilities
    • 1-2 bed adult family homes (AFHs)—owner occupied and corporate owned
    • 3-4 bed AFHs
    • Residential care apartment complexes (RCACs)
    • Community based residential facilities (CBRFs)

MCOs must follow the minimum fee schedule beginning October 1, 2024.


Rate increase

A 5% rate increase for home and community-based services was rolled out to eligible programs starting January 1, 2022. It applies to multiple service providers in Medicaid programs such as:

  • BadgerCare Plus Managed Care
  • Care4Kids
  • Children Come First
  • Children’s Long-Term Support (CLTS) Waiver
  • Family Care
  • Family Care Partnership
  • IRIS (Include, Respect, I Self-Direct)
  • Medicaid fee-for-service state plan services
  • Program of All-Inclusive Care for the Elderly (PACE)
  • SSI Managed Care
  • Wraparound Milwaukee
