ADRC: For Providers—Update or Add a Resource
This page is for resource providers that would like to be listed in the ADRC (aging and disability resource centers) statewide Resource Directory. The directory includes public, private, and nonprofit resources that relate to the health and wellness of older adults, adults with disabilities, their families, and caregivers.
We offer all resource providers the opportunity to request to be added to the statewide resource directory or to update their current information.
Update a resource
- Review the directory to see your current information listed.
- Complete the ADRC Statewide Resource Directory Request.
Add a resource
- Check the directory to ensure your resource is not already included.
- Review the Inclusion/Exclusion Policy, P-02923-11 (PDF) to determine if your resource qualifies for submission.
- Complete the ADRC Statewide Resource Directory Request.
Next steps
Once you submit your request, a team member will review it and reach out if additional information is needed or once the resource has been added.
Directory resources are reviewed annually to ensure accuracy and relevance. Your organization will be contacted by your local ADRC when updates to your information are required. If you do not respond to confirm or update your information, your resource may be removed from the directory.
