You may be able to temporarily enroll in Family Planning Only Services if you need family planning services right away and meet the eligibility criteria.
If you want to keep getting services once your temporary enrollment has ended, you will need to complete an application for ongoing Family Planning Only Services.
The Department of Health Services has approved certain family planning providers to help you temporarily enroll in Family Planning Only Services. You should check with your family planning provider to see if he or she can help you temporarily enroll.
If your family planning provider cannot help you temporarily enroll, you can call Member Services at 1-800-362-3002 and ask for a list of family planning providers near where you live who can help you temporarily enroll in Family Planning Only Services.
Your family planning provider will help you complete an application for temporary enrollment in Family Planning Only Services. You will need to give your provider information like the following:
- Full name
- Contact information
- Date of birth
- Address where you want to get mail about Family Planning Only Services
- Monthly income
You will also need to sign the application to confirm that the information you gave is accurate.
Once your provider submits your application, you will find out right away if it has been approved.
Temporary Identification Card
If your application for temporary enrollment is approved, your provider will give you a paper temporary identification (ID) card. You will need to show this temporary paper ID card when you have a family planning-related appointment or pick up a prescription for contraceptive supplies at a pharmacy. Your temporary paper ID card can be used for up to 14 days.
You will get a plastic ForwardHealth ID card in the mail soon after your temporary application is approved, unless you used to be enrolled in Family Planning Only Services, BadgerCare Plus, or Medicaid.
If you used to be enrolled in Family Planning Only Services, BadgerCare Plus, or Medicaid, you will not automatically get a new ForwardHealth ID card. You can use the same card you used before if you still have it. If you did not keep your ForwardHealth ID card, you can ask for a new card through your MyACCESS account or by calling Member Services at 1-800-362-3002.
Once you get your ForwardHealth ID card, you should sign it in the space provided. You should start using your plastic ForwardHealth ID card as soon as you get it.
Length of Temporary Coverage
Your coverage will start on the day you applied for temporary enrollment in Family Planning Only Services and will end the last day of the month after the month you started getting coverage. This means that if your coverage started on May 10, you would be covered until June 30.
If you apply for ongoing Family Planning Only Services and a decision is made about your eligibility for ongoing benefits, your temporary enrollment will end on the date of that decision.
