Lead-Safe Wisconsin: Acceptable Photo Identification for Lead and Asbestos Certification
The Lead and Asbestos Section of the Wisconsin Department of Health Services (DHS) may require you to submit a photo ID when you apply for lead or asbestos certification.
Below is a list of types of ID you can use if they contain a photograph and proof of birth date. If you don't have any of these, email the lead and Asbestos Certification Unit or call 608-261-6876 to learn what you can do.
- Tribal Nation document
- Driver’s license issued by a Canadian government authority
- Driver’s license or ID card issued by a state or outlying possession of the United States, as long as it contains a photo or information, such as a name, birthdate, sex, height, eye color, and address
- ID card issued by federal, state, or local government agencies or entities, as long as it contains a photograph or information, such as a name, birthdate, sex, height, eye color, and address
- IRS individual taxpayer identification number (ITIN)
- Military dependent’s ID card
- Permanent resident card with photograph (I-551)
- Prison ID
- Resident citizen in the United States ID card (INS Form I-179)
- School ID card with a photo
- Unexpired employment authorization card (INS Form I-688A)
- Unexpired employment authorization document issued by the INS that contains a photo (INS Form I-688B)
- Unexpired foreign passport with an I-551 stamp or attached INS Form 1-94, indicating unexpired employment authorization
- Unexpired temporary card (INS Form I-688)
- U.S. citizen ID card
- U.S. Coast Guard merchant mariner card
- U.S. military card or draft record
- U.S. passport (expired or unexpired)
