MCO Toolkit for Adult Long-Term Care Provider Enrollment
All providers of home and community-based services under an adult long-term care (LTC) waiver program must start enrolling with Wisconsin Medicaid through the ForwardHealth Portal right away. Providers who do not start enrolling now will not be able to deliver Medicaid services and their Medicaid claims will be denied. Managed care organizations (MCOs) are key partners to communicate this new requirement to providers.
We need your help. MCOs are required to share the following resources with providers to help us get as close to 100% enrollment by the new year as possible.
Who to share these materials with
The following providers and entities should enroll with Wisconsin Medicaid through the ForwardHealth Portal as soon as possible:
- Organizations and agencies
- Sole proprietors
- Individuals who are not self-directed support workers
Please note: individual self-directed support workers do not have to enroll with Wisconsin Medicaid. The requirement does not apply to them so they will keep working as usual.

Partners across Wisconsin are working together to help ensure all adult LTC waiver services providers start their enrollment right away. DHS is sharing enrollment progress so everyone knows how close to 100% enrollment the state is. Numbers are updated monthly.

You are receiving this email because our records indicate you have not started your enrollment using the ForwardHealth Portal.
Wisconsin Medicaid has changed provider enrollment requirements, and has asked you to self-enroll using the ForwardHealth Portal. You must start now, otherwise your Medicaid payments will end in 2026.
- If you are signing up with a new enrollment application, you may need to submit more than one. It depends on what services you provide and how many locations you have. The Online Handbook has more information. Search for Topic 23490 for information about enrollment by location.
- If you are updating your EVV-only enrollment to a full Medicaid enrollment, you should just do that with your current EVV-only provider Medicaid ID. You should not submit any new applications to create more Medicaid provider IDs.
- Once your application is approved, we must also authorize your services before you can be paid for Medicaid services.
- You cannot backdate your enrollment to a date before you submitted your application.
- You cannot hold Medicaid members and families liable for paying for Medicaid services you don’t get reimbursed for.

- You will lose business from people covered by Medicaid.
- Your contract with us will end.
- People who depend on you for vital services will have to find other providers.
- The provider shortage in Wisconsin will worsen, and remaining providers will experience more strain.

Medicaid payments to end for providers not self-enrolled through the ForwardHealth Portal
Wisconsin Medicaid has changed provider enrollment requirements. All our providers delivering Medicaid waiver authorized services must self-enroll with Wisconsin Medicaid through the ForwardHealth Portal. You must start your enrollment now, otherwise your Medicaid payments for services you deliver in homes or in the community will end in 2026. This means:
- You will lose business from people covered by Medicaid.
- Your contract with us will end.
- People who depend on you for vital services will have to find other providers.
- The provider shortage in Wisconsin will worsen, and remaining providers will experience more strain.
What you need to do
Most providers should submit a new provider enrollment application on the ForwardHealth Portal to get a Medicaid-issued provider ID.
- Go online at www.forwardhealth.wi.gov
- Click Become a Provider in the top left of the screen
Supportive home care agencies with EVV-only provider Medicaid IDs have a quicker process.
- They’ll convert their EVV-only enrollment to full Medicaid enrollment. It’s called revalidation.
- These agencies received a letter in the mail in August with instructions—they can simply log onto the ForwardHealth Portal and provide additional information after they get that letter. Then, they’ll be fully enrolled as a Medicaid provider. That means they will meet all state and federal requirements to keep delivering adult LTC waiver services to Medicaid members.
- They will keep the same provider ID after revalidation.
- This applies to agencies delivering services under service codes S5125, S5126, T1019, and T1020.
You must start now. Once your application is approved, we must also authorize your services before you can be paid for Medicaid services. Keep in mind, you cannot backdate your enrollment to a date before you submitted your application. And, you cannot hold Medicaid members and families liable for paying for Medicaid services you don’t get reimbursed for.
