Background check applications for all users

To prepare for the transition to the new DHS DQA Provider Portal, the Division of Quality Assurance (DQA) will not accept any new background check applications for entity operators, non-client residents, or other required individuals for new or renewing entity license/certification applications, or for rehabilitation review applicants as of November 22, 2025.

DQA will resume accepting background check applications on December 15, 2025, through the DHS DQA Provider Portal. To prepare for this transition and for more information, visit our webpage. Caregiver background checks completed by entities for employees and contractors are not affected by this transition.

In Wisconsin, all X-ray devices must be certified and registered.

Description

A portable X-ray services certification is for groups that supply portable X-ray services in the Medicare program.

A portable X-ray is X-ray equipment that you can carry with your hands. Providers can move the X-ray equipment as needed. Often, portable X-ray providers bring X-rays to people’s homes or group living settings.

The Division of Quality Assurance (DQA), Bureau of Health Services, is in charge of surveying portable X-ray providers. They also work with the Radiation Protection section to investigate complaints related to these providers.

Duration

One year. Wisconsin Stat. §§ 254.31 through 254.45 requires registration of all X-ray devices each year.
