Family Health Survey: Information for Researchers
The Family Health Survey questionnaire is administered over telephone by trained interviewers. The adult household member who self-identifies to know the most about the health of all household members responds for the whole household. Currently, the questionnaire includes the following modules:

- Health care utilization
- Health literacy
- Health conditions
- Health insurance
- Employment
- Demographics
- Income

Request Family Health Survey data
If you would like to request summary data tables on one of the topics that Family Health Survey collects, send us an email to dhsfhsdhs.wisconsin.gov with the details of your request.
If you would like access to Family Health Survey data to conduct your own analysis using appropriate statistical software, review the Technical Notes, P-45369b-21 (PDF), document and the relevant questionnaire and codebook above, and email us at dhsfhs@dhs.wisconsin.gov to get a data application.
The Division of Public Health Data Governance Board will review your completed application. Upon approval, you will sign a Data Use Agreement and receive access to the data you requested. You will be responsible for storing the data securely and using appropriate analysis methods.
