This toolkit was created to provide health plans and health care organizations with a guide for implementing home visiting services for asthma, the most common chronic condition affecting children. It provides information about the benefits of asthma home visiting and steps to develop and implement the program within health plans or health care organizations.

Asthma home visiting is an evidence-based strategy that is shown to reduce asthma-related hospitalizations, emergency department (ED) visits, and healthcare costs. The Wisconsin Department of Health Services’ (DHS) Division of Public Health (DPH) currently administers an asthma home visiting program called the Asthma-Safe Homes Program, which funds in-home asthma education and asthma trigger-related home repairs and remediation in several Wisconsin counties with high rates of asthma. To expand these services throughout Wisconsin, health plans and health care organizations can implement these services in their communities.
