MCO Toolkit for Adult Long-Term Care Provider Enrollment
All providers of home and community-based services under an adult long-term care (LTC) waiver program must enroll with Wisconsin Medicaid through the ForwardHealth Portal by January 1, 2026. Providers who do not enroll by the deadline will not be able to deliver Medicaid services and their Medicaid claims will be denied. Managed care organizations (MCOs) are key partners to communicate this new requirement to providers.
We need your help. 2026 is closer than it seems, and time is running out to process enrollment applications and authorize services by January 1. MCOs are required to share the following resources with providers to help us get as close to 100% enrollment by the new year as possible.
Who to share these materials with
The following providers and entities should enroll with Wisconsin Medicaid through the ForwardHealth Portal as soon as possible:
- Organizations and agencies
- Sole proprietors
- Individuals who are not self-directed support workers
As of August, this also includes supportive home care agencies and supportive home care agencies with an electronic visit verification (EVV)-only provider Medicaid ID.
Please note: individual self-directed support workers do not have to enroll with Wisconsin Medicaid. The requirement does not apply to them so they will keep working as usual.
Enrollment progress tracker
Partners across Wisconsin are working together to enroll all adult LTC waiver services providers by the January 1, 2026, deadline. DHS is sharing enrollment progress so everyone knows how close to 100% enrollment the state is. Numbers are updated monthly.

As you know, Wisconsin Medicaid has changed provider enrollment requirements. You must self-enroll with Wisconsin Medicaid through the ForwardHealth Portal by January 1, 2026, to remain a Medicaid provider and get reimbursed for Medicaid services.
As of August:
- Supportive home care agencies can now start a provider enrollment application.
- Supportive home care agencies with an electronic visit verification (EVV)-only provider Medicaid ID can convert their EVV-only enrollment to full Medicaid enrollment (revalidate).
You are receiving this notice because our records indicate you have not yet completed this requirement. Time is running out to process provider enrollment applications in time to avoid disruptions in the new year. Take some time in the next month to enroll.
We would like to remind you that:
- You cannot get paid for Medicaid services delivered in homes or in the community after January 1, 2026, if you do not enroll by the deadline.
- After you enroll, [we OR the IRIS consultant agency you work with] must also authorize your services before you can be paid for Medicaid services.
- You cannot backdate your enrollment to a date before you submitted your application.
- Medicaid members and families cannot be held liable for paying for Medicaid services you don’t get reimbursed for.

Those with an EVV-only provider Medicaid ID have a quicker process
Are you a supportive home care agency with an EVV-only provider Medicaid ID who provides services under service codes S5125, S5126, T1019, and T1020? You’ll convert your EVV-only enrollment to full Medicaid enrollment. You received a letter in the mail in August with instructions—simply log onto the ForwardHealth Portal and provide additional information after you get that letter. Then, you’ll be fully enrolled as a Medicaid provider. You will keep the same provider ID after revalidation.
