MCO Toolkit for Adult Long-Term Care Provider Enrollment

All providers of home and community-based services under an adult long-term care (LTC) waiver program needed to submit an application to enroll with Wisconsin Medicaid through the ForwardHealth Portal by December 31, 2025. Those who choose not to apply will not get reimbursed by Wisconsin Medicaid for dates of service on and after April 1, 2026.

Providers who want to keep delivering home and community-based services under one of Wisconsin adult long-term care programs can still apply, but they must apply now. Their application must be approved and contracts and services must be authorized before they can be paid by Wisconsin Medicaid. This can take several weeks. They may see a lapse in payment if their application is not approved by March 31, 2026.

Managed care organizations (MCOs) are key partners to communicate this with providers to minimize impact to Medicaid payments.

We need your help. MCOs are required to share the following resources with providers.

 Who to share these materials with

The following providers and entities should submit an application to enroll with Wisconsin Medicaid through the ForwardHealth Portal:

  • Organizations and agencies
  • Sole proprietors
  • Individuals who are not self-directed support workers

Please note: individual self-directed support workers do not have to enroll with Wisconsin Medicaid. The requirement does not apply to them so they will keep working as usual.

Enrollment progress tracker

Partners across Wisconsin are working together to help ensure all adult LTC waiver services providers start their enrollment right away. DHS is sharing enrollment progress so everyone knows how close to 100% enrollment the state is. Numbers are updated monthly.

Wisconsin Medicaid has changed provider enrollment requirements, and has asked you to apply using the ForwardHealth Portal by December 31, 2025. This means you will lose your contract with us and will not get reimbursed by Wisconsin Medicaid for dates of service on and after April 1, 2026.

How to apply

You should submit a provider enrollment application on the ForwardHealth Portal to get a Medicaid-issued provider ID.

  1. Go to the ForwardHealth Portal.
  2. Select Become a Provider.
  3. Follow the prompts to apply. You may need to submit more than one application. It depends on what services you provide and how many locations you have. The Online Handbook has more information. Search for Topic 23490 for information about enrollment by location.
  4. Once your enrollment is approved, you will receive a confirmation letter with your Medicaid-issued provider ID.

Reminders:

  • You cannot backdate your enrollment to a date before you submitted your application.
  • You cannot hold Medicaid members and families liable for paying for Medicaid services you don’t get reimbursed for.

  2. Select Start Provider Enrollment Application under Quick Links.
  3. Follow the prompts to apply.

Supportive home care agencies with EVV-only provider Medicaid IDs have a quicker process. They’ll upgrade their EVV-only enrollment to full Medicaid enrollment. It’s called revalidation. Use the EVV Revalidation Checklist (PDF) for help.

  2. Log in to your EVV secure Portal Provider account
  3. Select Revalidate Your Provider Enrollment and provide the information requested
