Wisconsin’s Mental Health and Substance Use Disorder Functional Eligibility Screen is a secure online screen to determine eligibility for community-based psychosocial rehabilitation programs for adults (18 and over) with mental health and/or substance use disorders. It can be completed for youth (16 and over) to allow for advanced planning.
