Southern Region Children’s Resource Center for Children and Youth with Special Health Care Needs (CYSHCN) Funding

Overview

The Wisconsin Department of Health Services (DHS) is seeking an organization to serve as the Children’s Resource Center for the Southern Region of Wisconsin to support Children and Youth with Special Health Care Needs (CYSHCN). CYSHCN are defined by the Health Resources and Services Administration (HRSA) as individuals aged 0—21 who have or are at risk for chronic physical, developmental, behavioral, or emotional conditions requiring services beyond those generally needed by children. The five Children’s Resource Centers, located in each of the state's five DHS regions, provide free and confidential services, via phone and email, that connect families and professionals with essential resources for CYSHCN.

The selected organization will provide localized support in the Southern Region to help families and professionals navigate complex medical diagnoses, learn about educational and community resources, find specialized provider networks, understand health insurance options, build peer networks, identify advocacy opportunities, and more. This project is funded by the Title V Maternal and Child Health (MCH) Services block grant.
