Guidance on the Transfer of Hospitalized Patients Infected with COVID-19 to Post-Acute and Long-Term Care Facilities

The purpose of this memo is to provide recommendations for the discharge disposition of hospitalized patients infected with COVID-19 who no longer require acute care and are ready for transfer to a post-acute and/or long-term care facility (PALTCF).
