BadgerCare Plus: Child Care Coordination

Child care coordination (CCC) is a Wisconsin Medicaid and BadgerCare Plus benefit for children under age 7 in Milwaukee County and children under age 2 in the City of Racine. To qualify, children must receive a CCC assessment within eight weeks after birth. The goal of CCC is to connect families to medical, social, and community services so kids can stay healthy and thrive.

Through child care coordination, you can choose a care coordinator who will:

  • Learn about your child’s current health, strengths, and needs.
  • Help you get the supports and services your family needs.
  • Respect your choices and ideas.
  • Meet with you at a time and place that you choose.

Once you have chosen a care coordinator, they will work with you to complete a questionnaire that looks at your child’s needs. Then, they will work with you to create a care plan that includes supports and services for your child and family. Care coordinators can:

  • Help you find a health care provider for your child.
  • Set up needed health care appointments for your child.
  • Give one-on-one support to identify and access resources to help you understand and deal with the challenges of parenting a young child, and help you develop a care plan.
  • Share information with you about helpful services in your community including FoodShare, housing, and transportation.
  • Talk with you about your CCC services and answer any questions you may have.
