Children’s Long-Term Support (CLTS) Provider Handbook
Welcome to the Children’s Long-Term Support (CLTS) Program!
As a CLTS Program provider, your role is vital in supporting children and their families by ensuring that each participant receives support and services tailored to their unique needs, strengths, and goals.
When you enroll as a Wisconsin Department of Health Services (DHS) CLTS Program provider, you become part of a collaboration among DHS, county waiver agencies (CWAs), CLTS participants, and their families.
- Participants are the center of the CLTS Program, along with their families.
- Families bring insight into the child’s strengths, preferences, routines, and goals.
- Providers bring their expertise and hands-on support to meet the needs of each participant.
- CWAs are experts on the program requirements. They authorize services, facilitate communication, and coordinate the team to ensure that services align with the participant-centered plan outcomes.
- DHS drives the vision as well as program direction and funding.
