Wisconsin Stat. § 251.06(3)(h) requires a local health officer to “submit an annual report of the administration of the local health department to the local board of health.” Wis. Admin. Code § DHS 140.04(2)(a) requires local health departments to submit their annual reports to the Department of Health Services (DHS) by May 1.
