Form a Leadership Team Worksheet

This is a worksheet from the Peer Professional Employer Toolkit. Print this worksheet and use it to help guide your organization in forming a leadership team.

  1. Team membership
    Determine who brings interest for peer recovery support services and who can bring specific expertise: an authorized leader; a human resources representative; a supervisor who champions the new service; and a service participant or member of the community. Consider repurposing an existing team to lead this project.





     
  2. Initial plan
    Review the introduction section and identify the current phase of your organization (exploration, preparation, or action). Within that phase, review and prioritize tasks to be completed. Then develop a plan for who will do what by when.





     
  3. Meeting frequency and agenda
    Given the initial plan, decide on meeting frequency to coordinate task completion. Identify standing agenda items for efficient and focused meetings.





     
  4. Communication plan
    Regular, transparent communication will be important throughout the implementation process with staff, supervisors, and upper management. Detail how the team will communicate with these groups to provide information and receive input during exploration (see summary of learning), preparation (see cultivate staff readiness, plan for integration), and action (see teaming integration).
