Children’s Long-Term Support: Waiver Renewal Family Input Sessions and Survey
The Wisconsin Department of Health Services (DHS) invites you to join an online input session in September to gather your ideas for how to improve the Children’s Long-Term Support (CLTS) Program!
DHS wants to hear directly from current and past participants and family members about their experience with the CLTS program and possible program changes. We hope to hear about experiences with different services, working with providers, ways to better serve certain groups with the program, and other program issues and solutions. Participants and families can register for a session to share their experience with the CLTS Program and ideas for the next five years of the CLTS Program. Each session will include a presentation about the CLTS waiver renewal project followed by a discussion with participants and family members. Your feedback will help DHS make the program better for children and families.

Sign language interpreters and captioning will be provided during the input sessions, and the presentation slides will be available in English, Spanish, and Hmong.
CLTS Program Family Survey
If you can’t join an online input session, you can also share your feedback with DHS by filling out the online family survey, which is also available in Spanish and Hmong. The survey is open until October 17.
