Children’s Long-Term Support (CLTS) Provider Handbook

Section 4: How to Become a CLTS Program Provider

There are two systems you will interact with regularly as a CLTS Program provider:

The CLTS Provider Registry

The Registry is where you will initially enter your contact information, the services you are qualified to provide, and the counties you serve. This is also where you will confirm and update this information in the future, as needed.

The ForwardHealth Portal

The ForwardHealth Portal is where you can verify participant eligibility, receive prior authorizations, submit claims, track payment activity, and receive updates. To get started accessing these systems, you will need a MyWisconsin ID.

MyWisconsin ID

A MyWisconsin ID is a secure login that lets you access Wisconsin online systems. All CLTS program providers must enroll in MyWisconsin ID. Providers can make multiple MyWisconsin ID accounts for employees that need to access their agency’s registration. If your agency decides to utilize one MyWisconsin ID account, DHS encourages you to use the director/CEOs information to register.

To get started, you’ll need:

  • A valid email address you can access.
  • Your legal first and last name.
  • A phone number for account security.

Providers can visit DET MyWisconsin ID Self Registration or call the MyWisconsin ID Account Service Desk at 608-471-6667 regarding any issues setting up the MyWisconsin ID and password. After receiving your MyWisconsin ID, you can begin enrolling in the CLTS Provider Registry.

CLTS Program provider directory and registry

The CLTS Program Provider Directory is a public-facing list of all qualified, enrolled providers in the CLTS Program. Being listed in the CLTS Program Provider Directory doesn’t mean you can deliver services right away. Services must be prior authorized. DHS encourages you to connect with CWAs in the areas in which you provide services. Support and service coordinators (SSCs) at the CWA match families and participants to providers during the service planning process and authorize services (see Section 6: Connecting With CWAs).

The purpose of the CLTS Program Directory is for participants and families, along with their SSCs, to locate providers who have met the state requirements and qualifications to provide supports and services in the CLTS Program.

All program providers must enroll through the online CLTS Provider Registry to be listed on the CLTS Program Provider Directory. The CLTS Program Provider Registry is where you will manage your CLTS Program enrollment, including service location, certification and licensing documentation, and other information.

You are required to submit the following information and documents during enrollment:

Once you submit your enrollment information, you will receive a confirmation notification. Once approved, the CLTS Medicaid Waiver Provider Agreement remains in effect for a maximum of three years.

Medicaid agreement

All CLTS Providers must sign and submit the Children's Long-Term Support (CLTS) Waiver Program Provider Agreement and Acknowledgement of Terms of Participation, F-03344. It takes 3–7 days to process from the date of submission. If corrections are required, you will receive a pending letter, and then you will have 10 business days to make necessary corrections.

What is a “willing” and “qualified” provider?

A willing provider is an individual or entity who:

A qualified provider meets the standards outlined in the service description and is verified through the qualification process completed by DHS. These providers are listed in the statewide public CLTS Provider Directory.

Your service category

The Medicaid Home and Community-Based Services Waiver Manual for the CLTS Waiver Program, P-02256 (PDF), Chapter 4.6, Allowable Services, lists the allowable CLTS services, qualifications, and limitations. It’s important to choose the CLTS allowable service that aligns with the service you are planning and qualified to provide. You will be responsible for submitting documentation of the qualifications required to deliver that service.

General information about provider licenses, education, and certifications

Each allowable service in the CLTS Program has unique specifications for licensure, education, and certification. For detailed information on the qualifications needed for each allowable service, see Medicaid Home and Community-Based Services Waiver Manual for the CLTS Waiver Program, P-02256 (PDF), Chapter 4.6, Allowable Services.

Choosing the counties where you will provide services

Providers must select a county or counties in which to provide services. Otherwise, your enrollment will remain in pending status.

When you enroll to provide services in certain counties, it means you are responsible for delivering or providing services in that county. If you provide services at one or more physical locations that participants travel to, list the county or counties of your physical location(s).

Some providers choose to provide services statewide, in all 72 counties. To provide statewide services, participants must be able to reasonably access the goods or services you offer. Providers enrolling to provide services statewide should either be:

  • Willing to deliver services in-person or remotely in all 72 counties.
  • Willing to ship a product to all 72 counties.
