BadgerCare Plus: Forms, Guides, and Publications
Keep getting helpful emails and texts
If you get a text or email message from Wisconsin DHS, take a moment to review and confirm your communication preferences. Complete this step to keep getting text and email updates.
This page is for our partner organizations—those who help BadgerCare Plus members. There are a lot of helpful materials about the program. We’ve added them here in one place. You can find:
- Forms—Files for members to fill out.
- Guides—Files that explain how BadgerCare Plus works.
- Publications—Files you can use to share details about BadgerCare Plus.
You can download most files in more than one language for members as needed. You also can print your own copies or order print copies. When you click the link, look at the Other Location column. Unless it says, “None,” you can order print copies.
Order BadgerCare Plus materials
Give these forms to BadgerCare Plus members (or potential members) if:
- They’re ready to apply for BadgerCare Plus.
- They want someone to act on their behalf. This can be a person or an organization. Learn more from the Representative Types Quick Reference, P-02176.
- They have a life change. This could mean moving, getting married, or having a child. Members must report changes to BadgerCare Plus.
- ACCESS (ACCESS in Spanish)
- Information Change Report, F-10183 (paper form)
- They need to allow someone to see information about their eligibility. Use the Release of Confidential Information Authorization for Wisconsin Medicaid, BadgerCare Plus, FoodShare, Family Planning Only Services, SeniorCare, and Caretaker Supplement, F-02340.
- They disagree with a decision made by BadgerCare Plus. They can request a formal hearing. Use the Request a Fair Hearing form.
- They live with kids under 18 years old. They must share details with BadgerCare Plus. Use the Statement of Identity for Children Under 18 Years of Age, F-10154.
