Construction Project Plan Review for Health Care Facilities: Construction Inspections
New online licensure and certification system for health and residential care providers
The Wisconsin Department of Health Services (DHS) Division of Quality Assurance (DQA) launched the DHS DQA Provider Portal, a new online system for licensure/certification, background checks, plan reviews, and other provider applications for certain users. Visit our webpage for more information.
Scam targeting U.S. Bank customers
DHS is aware of an organized impersonation scam that targets U.S. Bank customers. Impersonators are contacting customers by phone, posing as representatives from the U.S. Bank Fraud Department. They may claim issues with recently initiated wire transfers. The DHS DQA Provider Portal uses U.S. Bank for the payment of fees. Please be aware of this impersonation scam if you are paying fees through the DHS DQA Provider Portal.
Upgraded commercial building code took effect September 1
The Department of Health Services (DHS) will accept plans submitted under the current (2015) code through the month of October.
Starting November 1, all commercial health care building plans submitted to DHS for approval must meet the standards set in the upgraded 2021 Commercial Building Code.
Any supplemental sub-submissions to DHS, such as fire suppression/alarm, HVAC, etc., must be aligned with the code under which the commercial building plan was approved, regardless of how many months later they follow.
Please see the Department of Safety and Professional Services Wisconsin DSPS Provides Extension to Building Professionals (PDF) for more information.
Compliance statement
Facilities must complete and submit a Compliance Statement to the Division of Quality Assurance (DQA) prior to a final inspection. Compliance statements are required before occupying:
- A new building.
- An addition.
- Changes to an existing building.
Contact your DQA inspector with questions about final inspections.
Health care facility construction documentation checklist
The Health Care Facility Construction Documentation Checklist describes topics reviewed during an inspection.
Note: Meeting all requirements on these documents does not mean a facility meets inspection requirements. DQA has final inspection authority.
