Oral Health Program
Become an oral health screener for the Wisconsin Healthy Smiles Survey!
The Oral Health Program (OHP) will be conducting a Wisconsin Healthy Smiles Survey during the 2026-2027 school year to collect information on the oral health of ninth grade students. The OHP is actively recruiting licensed dentists and dental hygienists interested in serving as oral health screeners for this project.
Screeners will be responsible for communicating with school staff to schedule site visits, completing basic oral assessments on all participating ninth grade students, collecting data, and providing students with oral health supplies. Additionally, screeners are expected to adhere to all privacy laws, policies, and procedures. Screeners will be trained on survey protocols and will be compensated for all travel, work, and training sessions. One screener will be hired for each public health region.
Please note: If selected, an Employer Identification Number (EIN) and a Unique Entity Identifier (UEI) is required. If you do not have these numbers, it's strongly encouraged you start the process of obtaining an EIN and UEI. All applicants must not be disbarred, suspended, or ineligible.
