Guardians should be aware of specific rights a ward and/or their guardian has while the ward resides in and/or receive treatment from facilities regulated by the Division of Quality Assurance (DQA). Information is available through the DHS Client's Rights Office.

Change of address, status, or ownership

Corporate guardianship agencies must notify DQA corporate guardianship program (CGP) of changes in:

  • Capacity
  • Directory and counties served
  • Ownership
  • Program manager
  • Withdrawal/closure
