Consumer Guide: Consumer Information Reports for Nursing Homes and Facilities for People with Developmental Disabilities

Each year, the Division of Quality Assurance creates Consumer Information Reports. The reports have details about each Wisconsin nursing home and facility serving people with developmental disabilities (FDDs, also known as Intermediate Care Facilities for Individuals with Intellectual Disabilities, or ICF/IID). Reports mention any federal violations and staff information.

The goal of the reports is to help people who live or plan to live at these facilities. They can use the reports to judge the quality of care they will, or do, get.

How to use the Consumer Information Reports

We list the report for each nursing home or FDD at the bottom of this page. Facility names are in ABC order. You can search and filter the list by your city/town/village or county. You also can select a column heading to change the list order. When you find a facility to view, select the Publication Number link. It will take you to the most recent reports.

Note: Some nursing homes or FDDs may not have a report. This is because they are state licensed, which means they don't accept patients with Medicare or Medicaid. They also may be too new to have a report. We don't include facilities that have closed.

• Visit the nursing homes and FDDs that interest you. You can learn more and see a full directory on our Finding and Choosing a Nursing Home page and the Finding and Choosing a Facility Serving People with Developmental Disabilities page. You can also learn more about other types of assisted living.
• Use the Provider Search tool to find health and residential care providers in Wisconsin. You can filter results to show only certain types of facilities, such as nursing homes or FDDs.
• View facility ratings with the Centers for Medicare & Medicaid Services' Nursing Home Compare tool. It uses a five-star quality rating system for each nursing home. Ratings come from survey inspections, staff to patient ratios, and other aspects that define quality.
• Look at Residential Care Options by County. Learn about other ways to get health or residential care near you.
