FoodShare: Six-Month Reporting

If you are getting FoodShare, we may ask you to update your household information every six months. You can do so by completing a Six-Month Report.

The form shows your household's details, such as your address and your spouse's job. It also asks you to report any changes.

How do I complete a Six-Month Report?

We will mail you a FoodShare Six-Month Report and Instructions, F-16076, about a month before it is due. You can complete it:

  • Online: log in to ACCESS (ACCESS in Spanish).
  • By phone or in person: contact your agency.
  • By mail: return the form to the address listed in the box at the top of the form.

You may need to provide documents that confirm your information. Watch for a letter titled “Required Next Steps to Receive Your Benefits.” It will tell you what’s needed.

Timing

Complete your Six-Month Report right away! Don't wait until the end of the month.

You must complete your form and submit documents that confirm your information by the end of the month when the form is due.

What if I turn in my Six-Month Report late?

If you don’t complete all steps by the end of the month when they’re due, federal rules require that we reduce your benefits based on when we get the information. For example, if you don’t provide your documents until the 15th of the following month, you’ll only get about half of your FoodShare.

The letter asking you to confirm your information could have a different due date than your form. It may not be the date when you can keep getting full benefits.

Your benefits will end if your form and any needed documents are more than a month late. You’ll need to reapply to start getting FoodShare again.

Do all FoodShare members need to complete Six-Month Reports?

Some FoodShare members only need to renew their benefits every 36 months. To qualify, all adults in the household:

  • Must be age 60 or older, blind, or have a disability.
  • Can’t be earning money from a job or self-employment.

The household can include children, but no one can be a migrant worker. These members may need to complete a Six-Month Report sometime during those 36 months if something changes.
