EMS E-Licensing

The State of Wisconsin EMS Office manages all licensure and credentialing through our license management system, E-Licensing.

Access the E-Licensing System

Creating an account

If you do not have an existing EMS account you will need to create one in the Wisconsin E-Licensing system. Email is the primary method of communication between the EMS Section and the account holder. Please verify the email address you in your E-Licensing account is current and that you can access it. E-Licensing is viewed best in either Google Chrome or Edge.

 Wis. Admin Code DHS § 110.13(4) states “An EMS professional shall notify the department of any change in his or her name, address, or other information kept by the department within 30 days of the change, using the department's electronic licensing system or, at the department's request, submitting updated information to the department in paper form.”

If you have a Wisconsin E-Licensing account but are not able to access your account, you can recover your login and password information. For further assistance, contact the Wisconsin EMS Section.

Application processing timeframe

Wis. Admin Code § DHS 110.10(1), states “COMPLETE APPLICATION. The department shall review and make a determination on an application that has been completed in accordance with all of the department's instructions for completion within 60 business days of receiving the application. If the department approves the application, the department will notify the applicant and issue a license, certificate or permit. If the department denies the application, the department will notify the applicant of the reason for the denial and any appeal rights.“

Wis. Admin Code § DHS 110.10(2), states “INCOMPLETE APPLICATION. When an incomplete application is received, the department will notify the applicant of any deficiencies within 60 business days. If the applicant fails to respond to the notice and fails to complete the application within 6 months from the date of initial submission to the department, the application is void. The department will not take any further action on the incomplete application. To be considered further by the department, the applicant shall meet the eligibility requirements and submit a new application as required under s. DHS 110.06."

Application information will only be provided to the applicant.

The Wisconsin EMS Section has 60 business days to review complete applications. Applications are processed in the order they are received into the system. Due to the volume of applications submitted, the Wisconsin EMS Section is not able to provide status updates for applications until four weeks after submission.

Certification Reporting Application

This application assists you in entering your CPR/ACLS/PALS/PEPP certifications into your EMS account and document Wis. Admin Code DHS § 110.13(4m) and Wis. Admin Code DHS § 110.13(5) requirements.

Training Center Training Permit Application

This application assists you in obtaining your EMS Training Center training permit. A training permit granted by the department authorizes the training permit holder to participate in field and clinical training and to perform the duties of an emergency medical services practitioner at the practice level for which the permit is issued while enrolled as a student with the training center. Wis. Admin Code DHS § 110.15(2)(a)

EMT Training Permit Application

This application is for students enrolled in a current EMT course. If applying for an EMT training permit to serve as part of the required minimum ambulance staff, the individual has completed the first 46 hours of the initial EMT training or has current certification as an emergency medical responder and holds a training permit at the EMT level. Wis. Admin Code DHS § 110.15(1)(b)

Pre-Determination Application

This application is for an individual with a conviction record who applies for a pre-determination on their eligibility for certification or licensure, the individual shall submit a review fee of $50 with the department-approved application. Wis. Admin Code DHS § 110.06(1)(f) and Wis. Admin Code DHS § 110.066(1)(g), Wis. Admin Code DHS § 110.066(1)(g) 1 and Wis. Admin Code DHS § 110.066(1)(g) 2.

Emergency Medical Responder (EMR) Initial Certification Application

This application is for initial EMR students who have successfully completed an initial Wisconsin EMR course to apply for initial EMR certification. Wis. Stat. § 256.15(8) and Wis. Admin Code DHS § 110.06 (NREMT certification is not required.)

Emergency Medical Technician (EMT) Initial License Application

This application is for initial EMT students who have successfully completed an initial Wisconsin EMT course to apply for initial EMT license. Applicants must meet Wis. Admin Code DHS § 110.06 requirements.

EMS Practitioner Upgrade License Application

This application is for initial AEMT, Paramedic and Critical Care students who have successfully completed an initial Wisconsin EMS course to apply to upgrade a current license level. Applicants must meet Wis. Admin Code DHS § 110.08 requirements.

Reciprocity License Application

This application is for individuals who hold current EMR certification or EMS licensure granted by a state to obtain initial Wisconsin EMR, EMT, AEMT, Paramedic license. Applicants must meet Wis. Admin Code DHS § 110.066 requirements.

F-03230, Wisconsin Reciprocity Application Checklist (Word) This checklist will assist you in preparing complete the Wisconsin Reciprocity License Application and operational plan.

  • Verification of Licensure form - Wis. Admin Code DHS § 110.066(1)(c) states “The individual presents documentation of a current license or certificate issued by another state at or above the level being applied and a completed verification of licensure form from every state the individual has ever been licensed or certified as an emergency medical responder or emergency medical services
  • Verification of Education form - Wis. Admin Code DHS § 110.066(1)(d) states “The individual presents a completed verification of education form documenting completion of an initial course equivalent to the training required for an initial license or certificate under the applicable Wisconsin curriculum, as determined by the department. If initial training was completed prior to the current licensing period, the applicant also presents documentation of completion of an approved refresher course, as determined by the department.”
Local Credentialing Agreement Form

This form is for a current certified EMRs and licensed individuals with written authorization by the service director and medical director of a licensed emergency medical services provider permitting a certified emergency medical responder or licensed emergency medical services practitioners to perform specified emergency medical care while in the service of the provider. To be “credentialed" means to hold a credential issued by a licensed emergency medical services provider. The EMS service provider service director and medical director approve this form.

Registered Nurse Equivalency Form

This application is for a currently licensed Wisconsin registered nurse to obtain an equivalency status of an EMR, EMT, AEMT, Paramedic, or Paramedic with Critical Care endorsement. If you do not have an existing EMS account you will need to create one in the Wisconsin E-Licensing system. Completion of this application will not result in the granting of a separate EMS certificate or license but is used to add you to the roster in the E-Licensing and Wisconsin Ambulance Run Data System (WARDS). Completion of the application is required for equivalency of an EMS provider in the State of Wisconsin.

Physician Assistant Equivalency Form

This application is for a currently licensed Wisconsin physician assistant to obtain an equivalency status of an EMR, EMT, AEMT, Paramedic, or Paramedic with Critical Care endorsement. If you do not have an existing EMS account you will need to create one in the Wisconsin E-Licensing system. Completion of this application will not result in the granting of a separate EMS certificate or license but is used to add you to the roster in the E-Licensing and Wisconsin Ambulance Run Data System (WARDS). Completion of the application is required for equivalency of an EMS provider in the State of Wisconsin.

This application is for a currently licensed Wisconsin physician to obtain an equivalency status of an EMR, EMT, AEMT, Paramedic, or Paramedic with Critical Care endorsement. Completion of the application is required for equivalency of an EMS provider in the State of Wisconsin.

Equivalency Local Credentialing Agreement Form

This form is for a current licensed Wisconsin registered nurse, Wisconsin physician assistances and Wisconsin physicians that have been granted equivalency to be added to the roster of a licensed service provider to function as an EMS professional for the service provider. The EMS service provider service director and medical director approve this form

Pre-Determination Application

This application is for an individual with a conviction record who applies for a pre-determination on their eligibility for certification or licensure, the individual shall submit a review fee of $50 with the department-approved application. Wis. Admin Code DHS § 110.06(1)(f) and Wis. Admin Code DHS § 110.066(1)(g), Wis. Admin Code DHS § 110.066(1)(g) 1 and Wis. Admin Code DHS § 110.066(1)(g) 2.

Adding a driver to an EMS service provider roster

This form allows an Emergency Medical Service Provider Service Director, to add a non-EMS-trained driver to the service provider roster in the Wisconsin E-Licensing system. If you do not have an existing EMS account you will need to create one in the Wisconsin E-Licensing system and the Wisconsin Ambulance Run Data System Elite (WARDS).

Adding a billing representative to an EMS service provider roster

This form allows an Emergency Medical Service Provider Service Director, to add a billing representative to the service provider roster in the Wisconsin E-Licensing system. If you do not have an existing EMS account you will need to create one in the Wisconsin E-Licensing system and the Wisconsin Ambulance Run Data System Elite (WARDS).

EMS Practitioner Upgrade License Application

This application is for initial AEMT, Paramedic, Critical Care students who have successfully completed an initial Wisconsin EMS course to apply to upgrade a current license level. Applicants must meet Wis. Admin Code DHS § 110.08 requirements. This application is also used for Tactical EMS (TEMS) endorsement.

Changes and updates to an existing EMS Service Provider Operational Plan

If you are planning on submitting an update in the emergency medical service provider operational plan, you will want to use the following resources to assist you:

  • F-03135, Update to An EMS Service Providers Operational Plan Checklist (Word) This checklist will assist you in identifying and preparing documentation to complete the update to the service provider application and operational plan. You will not need a feasibility study to update your service provider application and operational plan. You only need a feasibility study for initial or upgrading an EMS service provider license.

Change of EMS Service Provider Service Director

Change of EMS Service Provider Service Medical Director

If you are changing an emergency medical service provider medical director, please complete a F-47470 Change of EMS Medical Director form (PDF) and return it to the EMS Section for processing.

Adding a vehicle to an EMS service provider

This form allows an Emergency Medical Service Provider Service Director, to add a vehicle to the service provider in the Wisconsin E-Licensing system.

EMS Training Center Initial and Renewal Application

This application is to assist in creating or making changes to an existing training center as required under Wis. Admin Code § DHS 110.18(2).

EMS Training Center course approval requirements

This application is to assist in creating or making changes to an existing training center Wis. Admin. Code DHS § 110.19 states “A training center shall obtain prior department approval for each initial or refresher course it offers. Once a training center obtains approval, all subsequent course offerings based on that approval are automatically approved when entered in the department's training database, provided the training center has renewed its certification under Wis. Admin. Code DHS § 110.18 (3). If the training center changes any component of an approved course, it shall submit the change to the department for approval prior to implementation.”

Instructor certification applications

Instructor II Certification Application

As outlined in Wis. Admin. Code § DHS 110.28, before you may act as an Instructor II, you will need to complete the application process. In doing so, you will have to identify a training center to assist you with the application process because you need to be certified and credentialed with a Wisconsin-approved EMS training center to be an instructor. Certification for EMS instructor II shall expire on June 30 of the 3rd year of the triennial period unless renewed. If an individual does not timely renew his or her EMS instructor II certification, under s. DHS § 110.28 (2)

Instructor II Local Credential Agreement

This form is for current certified Instructor II with written authorization by the training center program director and medical director to function as an Instructor I for the training center and to be added to the training center roster of instructors.
